Mr. Jerry W. Smith age 66 of Salem passed away Thursday, June 8 at Meadow View Health Care Center in Salem.
Mr. Smith was born May 26, 1951 in Salem the son of Otis and Leona Willis Smith. He was a former employee of Holms Industries.
He is survived by his wife of 29 years: Lana Schmidt Smith of Salem, a son: Travis Smith (fiancé, Chelsea Brewer) of Salem, two daughters: Bethany Walton (Sean) and Brittany Smith (fiancé, Tyler Banschbach) both of Salem, three sisters: Edith Wagner of Memphis, Indiana, Anita Carter (Charles) and Wanda Powers (Larry) both of Salem.
He was preceded in death by her parents, a brother: Clifford Smith, and a sister: Yvonne Amick.
Funeral services will be held Monday at 11:00AM at Weathers Funeral Home. Cremation will follow. Visitation will be Sunday from 4-8PM and Monday from 9AM-time of service.
